Directions
  1. Contact player's PRIMARY coach to receive permission to use player as a guest.
  2. Secure PRIMARY player's US Youth Soccer OR USASA Player Pass. FORM NOT VALID WITHOUT PLAYER PASS.
  3. Fill out the entire form below, all fields must be completely and accurately filled out.
  4. Click on submit at the bottom of the page (this sends a copy to the Nebraska State Soccer Association and also displays your official Guest Player form).
  5. Once your Guest Player Form loads- Print the form.
  6. Sign and date the form, and take the form to the tournament check-in.

Use this form only if the Guest Player you are taking is from Nebraska!
