Please Note: A $15 fee is due/postmarked within ten (10) business days of submission of this form. If payment is not received or postmarked within ten (10) business days you will be invoiced for the original amount plus a $25 late fee.
Directions Important Note: You can only enter letters (A-Z) or numbers (0-9) in the fields except for the phone fields. Skip any other characters when entering information. Fill out the entire form below, all fields must be completely and accurately filled out according to examples. Click on submit at the bottom of the page (this sends a copy to the Nebraska State Soccer Association and also displays your official Notification of Travel form). Once your Notification of Travel Form loads- Print the form and receipt page.Take the Receipt Page and mail with check to Nebraska State Soccer. Sign and date the form, and take the form to the tournament check-in.
You must fill out one form for each tournament you attend.
